    E-Sign Consent & Disclosure

    Last Updated: May 21, 2026

    1. Purpose and Scope

This E-Sign Consent and Disclosure ("Disclosure") explains how Pathora Inc (d/b/a "MediFlow 360," "we," "us," or "our") will deliver records to you electronically and how you may use electronic signatures on the MediFlow 360 platform. It is provided under the federal Electronic Signatures in Global and National Commerce Act, 15 U.S.C. §§ 7001 et seq. ("E-SIGN Act"), and the California Uniform Electronic Transactions Act, Cal. Civ. Code §§ 1633.1 et seq. ("UETA").

By creating an account, electronically signing a document, or clicking a button indicating your agreement, you affirmatively consent to (a) receive Communications (defined below) electronically, and (b) use electronic signatures in place of handwritten signatures.

    2. What Is Covered ("Communications")

Your consent applies to all records, notices, disclosures, agreements, and other documents we provide to you in connection with the platform, including but not limited to:

    • Terms of Service, Privacy Policy, HIPAA Notice of Privacy Practices, and any amendments.
    • Medical liens, lien acknowledgments, assignments, and related personal-injury case documents.
    • Patient intake forms, consent-to-treat forms, and authorizations for release of records.
    • Invoices, statements, receipts, and payment authorizations.
    • Service updates, security notifications, and customer support communications.
    • Any other document we are required or choose to provide to you in writing.

    3. Method of Delivery

We may deliver Communications by (a) posting them within the MediFlow 360 platform, (b) emailing them to the address associated with your account, (c) sending them by SMS to your registered phone number, or (d) any other electronic means reasonably designed to provide actual notice. It is your responsibility to keep your contact information current — see Section 6.

    4. Hardware & Software Requirements

To access and retain Communications, you will need:

    • A device (computer, tablet, or smartphone) with a current operating system that is actively supported by its manufacturer.
    • A current version of a modern web browser (Chrome, Safari, Edge, or Firefox).
    • A working internet connection.
    • A valid email address and (if applicable) SMS-capable mobile phone.
    • Software capable of viewing PDF files (such as a built-in PDF viewer or Adobe Acrobat Reader).
    • Sufficient local storage, a printer, or another means to save or print Communications for your records.

If these requirements change in a way that creates a material risk you will not be able to access or retain Communications, we will notify you and give you the opportunity to withdraw consent without consequence.

    5. Withdrawing Consent & Paper Copies

You may withdraw your consent to receive Communications electronically at any time by emailing support@mediflow360.com with the subject "Withdraw E-Sign Consent." Withdrawing consent will not affect the legal effectiveness, validity, or enforceability of electronic Communications or signatures provided before withdrawal.

After you withdraw consent, we may be unable to continue providing certain services through the platform and may suspend or terminate your account.

You may request a paper copy of any Communication previously delivered electronically by emailing support@mediflow360.com. There is no fee for paper copies, although we may charge a reasonable fee for repeated or bulk requests as permitted by law.

    6. Keeping Your Contact Information Current

You agree to promptly update your email address, mailing address, and mobile phone number in your account settings or by emailing support@mediflow360.com. We are not responsible for Communications that fail to reach you because your contact information is incorrect or out of date.

    7. Electronic Signatures

When you type your name, click "I Agree," "Sign," "Submit," or a similar control, or otherwise indicate assent on the platform, you are providing an electronic signature that has the same legal effect as a handwritten signature. Electronic signatures may be used for, among other things, medical liens, lien acknowledgments, assignments of benefits, intake forms, authorizations, and any other platform document.
